MEDICAL FORM INFO
All "Athletic and Medical Clearance Forms" must be completed and turned in before the first official day of practice on Feb 4th. Forms can be downloaded from the Freedom Website under athletics.
If you have already turned in a athletic clearance and medical form to the training room, complete the "Emergency Medical Authorization" Form ONLY and bring it to the training room to be stamped.
Dual Meet & Invitational Bus & Parent Transportation
We have BUS for the League Championship at Heritage and the Mudville Invitational. The number of student athletes participating in track exceeds the number of seats on the bus. To fill the gaps in rides, we are seeking parent drivers to allow as many athletes to go and compete. Without sufficient parent drivers, I will NOT me able to take ALL athlete’s to the meets and some athletes that have been working hard will not be able to make the trip. Decision for who will attend will follow: 1) Top Varsity competitor via performance marks which will give us the best opportunity to win the team portion of the meet and 2) Practice attendance.
If you are available to drive, please complete the Liberty Union High School District Field Trip by Private Vehicle Information on Freedom Athletics Web Site and return it to me along with your insurance documentation. I MUST HAVE ITBEFORE THE MEET.
Parents may drive their student without completing this form; however, NO OTHER STUDENTS MAY RIDE WITH YOU UNLESS THIS PAPER WORK COMPLETED AND ON FILE WITH THE ATHLETIC DEPARTMENT.
